With the Ministerial Decree of 16th April 2021 some important measurements are introduced.
With a specific notice of the General Directorate of Prevention, Minestry of Health, a digital passenger locator form (Passenger Locator Form – PLF) will be introduced to be shown before boarding, which will replace the self-declaration in paper.
The ordinance of April 2, 2021 has already been updated to the latest list of countries by Attached file 20 of the Dpcm March 2, 2021.

Anyone who enters for any period of time the National territory from foreign states or territories referred in lists B,C,D and E of annex 20, before entering the national territory, must fill in a specific digital form of localization. The same replaces the self-declaration referred in art. 50, paragraph 1, of the decree issued by the President of the Council of Ministers on 2nd March 2021, which can be provided with the terms here described as an alternative to the digital locator form only in the event of technological impediments.
Provided that no symptoms of COVID -19 arises and considering that the digital passenger locator form is compulsory as per art. 3, the arrangements of the present article do not apply to crew members or drivers of means of transport for passengers or goods, despite the obligation to take a molecular or rapid test, at the arrival at the airport, port, or borders, where possible, or within 48 hours from the arrival in the national territory at the local health authority of reference.

Travelers must register to the EU dPLF web application by providing a valid e-mail address and password.

- Travel companions
- Emergency contact information
- STEP 3: review and submit the dPLF
Before you submit the form, you will be able to preview the data you have insert and edit them if necessary. Once completed the PLF press the submit button.
You will receive a confirmation e-mail upon submission of the dPLF.

You will receive by email the dPLF with the QR code (The mail will contain a QR code and a link to download the dPLF in PDF).

- What is a digital Passenger Locator Form and how data are used?
Passenger Locator Forms (PLFs) are used by public health authorities to facilitate contact tracing in case travellers are exposed to an infectious disease during their travel by airplane or aircraft, ship (cruise/ferry), rail, bus or automobile. Information that travellers provide in PLFs can be used by public health authorities in destination countries to rapidly contact travellers, with the goal of protecting the health of travellers’ and their contacts, as well as preventing further disease spread.
Countries that require completion of PLF are currently using paper-based passenger locator forms or have developed national online systems. In light of the COVID-19 pandemic, a web application for a EU digital Passenger Locator Form (dPLF) was developed to facilitate the use of PLFs during cross-border health threats. This digital format allows for easier and more rapid data collection and exchange between stakeholders, making contact tracing more effective and efficient. - Who has to complete the digital Passenger Locator Form?
Travellers entering a European Union Member State via air, maritime (cruise or ferry) or ground crossing (bus, train, car) may be required to complete a digital Passenger Locator Form (dPLF). In case a family is travelling together, only one member of the family will have to complete the PLF. The details of the rest of the family members should then be included in this PLF. The same applies to groups travelling together. Only one person from the group should complete the PLF and the details of the rest of the group members should then be included in this PLF. Note that this applies only to families and groups of people that will take part in the same activities and stay at the same place during their visit to the destination country.
